Your Office Visit
To facilitate your office visit, we ask that you arrive at least 30 minutes
before your scheduled appointment. All co-pays and deductibles are due at the
time of service. In addition please bring the following:
- Any completed forms downloaded from this site or mailed to you from our
- Your insurance card and driver's license
- All imaging done outside of a Baptist Health Facility
- List of your current medications
- If your insurance requires a referral, please contact your primary

As a courtesy to our patients, we ask that should you need to cancel
your appointment, please give our office a 24 hour notice.
Our office is happy to offer professional assistance in filing your
insurance. Our physicians accept most major insurance plans including
Medicare, Blue Cross, United Healthcare and many preferred provider insurance
plans. Please verify participation with your insurance when making your initial
appointment. You will be responsible for obtaining referrals from your
primary care physician if required by your insurance.Billing
For all office visits and procedures, payment is required at time of service.
Co-pays and or coinsurance for those patients with Medicare or one of our
contracted managed care plans are also required at the time of your
Additional charges may be generated for procedures and tests. PSC
will bill your insurance carrier for expected payment. You will be responsible
for any deductible and/or co-pay amount.
If it is determined that surgery
is necessary, payment plans can be arranged prior to surgery if necessary. Did

Medications will not be refilled on weekends or after business
The need for surgery will be determined by our physician and scheduled at a
later date. However, we do minor office procedures in our clinic. These
in-office procedures do not require you to be fasting after midnight.
